Requesting an Education Health and Care Needs Assessment (EHCNA)

Requests for a needs assessment can be made by anyone who knows the child and thinks an assessment may be necessary, including doctors, health visitors, Early Years settings and parents.

Parents/Carers

If parents make the request for an EHCNA please use the form below.

If the parent makes a request the setting should also submit the Early Years setting request form below, preferably at the same time as the parent request.

If schools and colleges are making the request please complete the Setting Request for an Education, Health and Care Needs Assessment form.

EHCP-Annual Review

EHCP Annual Review - all children with an EHCP must have a review every 6 months if under age five - settings have a responsibility to plan and organise the annual review.

 KEY DATES

• 6 weeks before the meeting: send out invitations, contact SEND for a word version of the EHCP for editing
• 2 weeks before the meeting: circulate reports
• 10 days after the meeting: send EYFS annual review report form and any reports received to to SEND team

Documents to support the annual review

These are the documents you need to prepare for an annual review and complete following the annual review:

• Annual review model invitation
• Agenda person centred review agenda 
• Parent's contribution template  to be sent to parents for parents to complete prior to review 
• EYFS annual review report form to be returned to SEND together with an annotated EHCP outlining changes to be made and all reports submitted.
